We are conducting a survey of nurses and nursing students related to interprofessional dysphagia management and kindly ask for your participation.

The survey is voluntary and completely anonymous, and should take about 10 minutes to complete. We ask that you complete the survey by September 30th, 2018.

At the end you will have a chance to enter a drawing for a $25 Amazon gift certificate. 1 out of every 50 participants will be chosen at random. Contact information for the drawing will be collected and stored separately from survey responses.
